It takes a village to deliver the best outcomes for every patient, getting patients home sooner without the risk of relapse or collaborating with their behavioral healthcare team if they return to the ED in crisis. To achieve this goal, every data door across the healthcare continuum must be open and able to freely exchange patient information.
The ability to attain greater transparency during patient transitions and see patient activity across the continuum of care delivers better healthcare outcomes for all. Achieve faster insights, work more collaboratively, and open the doors to interoperability, helping caregivers do what they do best—in every care setting.
This collaborative care network approach strengthens relationships between health plans, hospitals, Medication Assisted Treatment (MAT) facilities, post-acute care facilities, and other behavioral and mental health providers to deliver consistent evidence-based care, while maximizing the impact of a limited behavioral health workforce.
This session explores ways to improve communications across the siloed healthcare ecosystem, and how a more collaborative approach to addressing mental and behavioral health can improve outcomes and reduce costs. From the real-world case example of patient data insights for emergency department clinicians to data exchange between behavioral healthcare providers and hospital case managers, attendees will learn new ways to streamline clinical workflows, reduce length of stay, improve patient outcomes, and trim healthcare costs.
